Original Tender Description

To provide care and support for individuals who are accessing support from the Alcohol Care Team (ACT) based in the University Hospital of North Tees, to form the liaison between NTH acute service and local community substance misuse service in Hartlepool - START. To utilise START reporting system - Systmone to assess if individuals are known to START and ascertain current treatment plans. To work collaboratively with START and allocated Recovery Coordinators from the service to plan joint care and risk management plans delivered within START core hours, including ensuring START are aware of any admissions and also discharge planning; including commencement of Opiate Substitute Therapy (OST), and ensuring Naloxone provision has been given where applicable. The provider will work with START to ensure there is agreed performance reporting to the quarterly substance misuse governance group
