Schema teikia laikinas apgyvendinimo paslaugas ligoninėje besigydantiems pacientams vertinimui ir reabilitacijai, taip palaikydama stabilius būsto sprendimus ir pagerindama pacientų srautą iš ligoninės bei mažindama pakartotinio hospitalizavimo rodiklius. Paslauga susideda iš keturių butų visoje savivaldybėje, siūlančių iki šešių savaičių apgyvendinimą tiems, kurie negali grįžti namo tiesiai iš ligoninės. Šis palaikymas taip pat apima išsamią ilgalaikių poreikių vertinimą ir padeda spręsti tokius klausimus kaip būstas, skolos ir sveikata. Ieškome paslaugos teikėjo.
