Provision of Social Care By the Community Sector

The Council is using the RESERVED Contracts Criteria only suppliers who meet this will be have their applications considered The provision of social care service to adult residents with non-complex care and support needs. The three elements within the required adult social care service are as follows: a)Information, Advice and Guidance (IAG); b)Care Act 2014 assessment; •Care Act Assessment •Financial Assessment •Support plans c)Responsibility for statutory care coordination including named worker and Care Act review. The Contractor must be able to provide all three elements of the service
