Supported Living Services for Adults with a Learning Disability

This service will deliver medium to high intensity accommodation based housing related care and support and will meet the individually assessed needs of the people who will receive the service, as reflected through their person-centred support plans The accommodation consists of two 3 bed-roomed ground floor flats which are designed to be fully accessible for people who have physical disabilities, and a further 10 single bedroom flats, accessible for people who have physical disabilities, sited on the first and second floors of the buildingThe Service Provider will sign a Service Level Agreement and Licence to Occupy with the landlord RSL, A2 Dominion Housing.The Service Provider will provide an appropriate pool vehicle within the scheme for use of its staff to transport service users to appointments and activities
