Provision of Social Inclusion

Hywel Dda University Local Health Board has awarded contracts for the provision of Social Inclusion. This service is for all ages and across the Hywel Dda health board footprint. The aims of the service are: Social Inclusion services are to support those who are experienced Mental ill Health and who are prone to poor mental health concerns, loneliness, isolation, and poor self-esteem. Social inclusion enables a more positive and healthier human experience. Providing a service which supports the needs of those at the risk of a social/emotional crisis Act as a conduit to identify, broker, and signpost individuals to other help and support agencies as needed
