Provision of a Transitional Support Service in a Residential Care Setting

The Borough Council of Calderdale (the Council) invites Tenders from Tenderers who wish to be considered for the provision of a Transitional Support Service in a Residential Care Setting (the “Service”). It is anticipated that the contract will commence on the 1st April 2023 for a period of three (3) years with an option to extend for two (2) further periods of 12 months.The aim of the Service is: (1) To provide an enhanced, personalised transfer of care pathway from hospital to home for Individuals who are recovering from a period of ill health or who need a further period of convalescence and reablement whilst their long-term support needs are assessed.(2)To provide a step-up service to support people who have had a period of ill health or crisis within the community and require convalescence and reablement until they can return to their own homes. (3) To reduce or delay the need for long term care or avoidable admission / readmission to hospital.
